Addresses should be an address (or preferably a group of addresses) external to the firewall, usually the default gateway. You should also set up a second link monitor with internal addresses, and possibla additional ones for each interface you want to monitor.

1b. Since more than 3/5 of the hearbeats still gets through there will be no failover when one link fails. You need to set up a link monitor with action failover.
